Tools for Managing Your Care
During your cancer treatment, you’ll have a team of caregivers — your doctor(s), an oncology nurse, a pharmacist, financial counselors and others. You’ll also receive a lot of information about your disease and its treatment.
   
We recommend keeping that information organized in a folder or three-ring binder so you and your family can stay on track. Include:
  
• Copies of medical forms
• Contact information for your caregivers
• Test results
• Insurance information
• Explanation of Benefits forms from your insurance company
• Statements from various service providers
• Notes about questions you want to ask or things to remember from your appointments
