This oncology-specific disease management program increases treatment compliance, helps patients monitor and manage side effects, and reduces unnecessary ER visits and hospitalizations. It is designed to prevent expensive, life-threatening complications before they occur. As soon as patients begin treatment, they receive a call from a nurse to check on their status and monitor side effects. The call, which is repeated periodically while the patient is undergoing treatment, is an opportunity to make sure patients have answers to their questions and continue taking their medications properly.
Integrated with the oncology practice, our program provides a standardized patient education plan and a targeted side effect profile. As more patients survive longer with cancer, the program may help improve overall health and quality of life for long-term survivors.
