Care Management Tools
During cancer treatment, patients will have a team of caregivers — doctor(s), an oncology nurse, a pharmacist, financial counselors and others. They will also receive a great deal of information about cancer and its treatment.
 
Keeping information in a folder or three ring binder can help patients and family members stay organized. Some items to include:
 
•Copies of medical forms.
•Contact information for your caregivers.
•Test results.
•Insurance information.
•Explanation of Benefits forms from the insurance company.
•Statements from various service providers.
•Notes about questions to ask or things to remember from appointments.
